We do accept purchase orders from universities, government organizations and large business entities! To complete a purchase order simply fax your purchase order request to 800-494-1340. Please be sure to include an email address and phone number for quick correspondence with your P.O.
Steps to complete your purchase order:
- Find the items you would like to purchase and add them to your cart or add them manually to your purchase order. It is not necessary to include the product code with your PO line items, but it is helpful. All prices online are current.
- You can print the shopping cart screen and include that with your purchase order or add the line items manually to your PO.
- Before faxing your PO please include:
- Products you need including product code, product name, price and quantities.
- Valid Shipping Address with any room numbers, suite numbers, etc.
- Valid Bill to address where we can send the bill for the purchase order.
- Contact person name, phone number, and fax number.
- Fax your purchase order to 800-494-1340. You can also email your purchase order to:

Need a bid?
Some institutions require a bid before submitting a purchase order. We would be happy to provide a bid for the products you need. Please be sure to include product codes and/or product names and we will send a bid to you promptly.
W9 Form Request
Do you need a W9 Form on file for Visu Medical? Simply call, email us, or submit your request via fax and we will send it right over to you.
Tax Information
If you are outside of Kansas, we do not charge sales tax so there is no need to provide tax exempt form unless your institution is located in Kansas. **NOTE: Depending on your federal and state tax guidelines, you may be required to add sales tax to your purchase order.
